How to Complete the Assessment

To complete a WellCheck assessment:

  • Open the email notification you receive

  • Follow the link to the assessment

  • Answer the questions in your own words

  • Submit the assessment when complete

You do not need to worry about formatting, grammar, or structure. Clear, direct responses are sufficient.

What Happens After Submission

After you submit:

  • Your responses are reviewed alongside public data

  • A WellCheck summary is generated

  • Both you and the donor receive the summary
